What is 2-Deoxy-D-glucose
or 2-DG?
2-DG is a glucose
molecule in which the 2-hydroxyl group (OH) substituted by hydrogen (H) so that it
cannot undertake further glycolysis. It helps competitively to prevent the
production of glucose-6-phosphate from glucose at the phosphoglucoisomerase
level of the glycolysis process. Its other names are 2-Deoxyglucose,
2-Deoxy-d-mannose, 2-Deoxy-d-arabino-hexose, 2-DG.
Although no new
anti-COVID medications are available in India or internationally, medications
such as Remdesivir, Tocilizumab, Steroid drugs, Ivermectin, Hydroxychloroquine
(until last year), and Dexamethasone are being reconfigured for use in taming
the effects of the virus. Moreover, their utilization and affordability have
been called into question in recent days. In this situation, researchers
believe that the 2-deoxy-D-glucose, therapeutic COVID compound is much superior
to the repurposed medications.

Mechanism of
Action of 2-DG against SARS-CoV-2 or Coronavirus:
2-Deoxy-D-glucose or
2-DG is up engaged by the glucose transporters of the cell. As a result, cells
with higher glucose uptake, for example, tumor cells or virus-affected cell,
have also a higher uptake of 2-DG or 2-Deoxy-D-glucose. 2-DG or
2-Deoxy-D-glucose is a glucose analog molecule, which looks like a glucose
molecule but it is not glucose. A virus or bacteria that are reproducing fast
in the body needs glucose for energy. Thus, the virus or bacteria will take
this glucose analog and will get arrested or stop multiplying. This is how this
drug will help to stop the virus from multiplying in the body of an organism.

Controller General of India approved 2-Deoxy-D-glucose or 2-DG an anti-COVID
oral drug, developed institute of Nuclear Medicine and Allied Sciences (INMAS),
New Delhi, a lab of the Defense Research and Development Organization, in
collaboration with Hyderabad-based pharma company Dr. Reddy’s Laboratories for
emergency use as an adjunct therapy in moderate-to-severe coronavirus patients.
In the clinical trials, data suggest that the drug is effective in cutting
oxygen dependence. It was experimental that the 2-deoxy-D-glucose (2-DG)
molecule works efficiently against the SARS-CoV-2 virus and inhibits viral
growth.
Clinical Trials of 2-deoxy-D-glucose and accuracy of vaccine
Based on each of these results, the Drugs Controller
General of India’s (DCGI) Central Drugs Standard Control Organization (CDSCO)
has approved a Phase II clinical trial of 2-DG in COVID-19 patients beginning
in May 2020. The DRDO and its business partner DRL, Hyderabad, have started
clinical trials to evaluate the drug’s safety and efficacy in COVID-19
patients. The drug was shown to be stable in COVID-19 patients and demonstrated
substantial progress in their rehabilitation in Phase-II experiments (including
dose-ranging) performed between May and October 2020. The Phase II clinical
trial was carried out in six hospitals, and the Phase IIb (dose-ranging) clinical trial was carried out in 11 hospitals around the world. A phase-II
experiment of 110 clinicians was carried out.
Can all COVID-19 patients use 2-deoxy-D-glucose?
Currently, the medication
is only approved for use in mild to extreme COVID-19 situations. Patients with
minor infections that are healing at home do not need the opioid and can safely
heal at home if proper procedures are followed. Know that, like many others,
this is also an experimental drug with emergency use approval. The continued
use of the medication (or any other drug), as well as stockpiling stocks,
without prior approval by the prescribing doctor, would only lead to
complications
Dose of medication of 2-deoxy-D-glucose for COVID-19 patients
The medicine, which is
being co-manufactured by Dr. Reddy’s Laboratories, has yet to be priced.
